You should use this form, of you are seeking an independent review of a decision made by the Director of Civil Aviation that affects you. IMPORTANT: An application for review must be made within 20 working days of you being notified of the decision by the Civil Aviation Authority. If this form is not submitted within that 20-working day period, your application for review will be declined.

Documents required

At the first instance, you must provide related documents that detail the aspects of the decision you are seeking to have reviewed, the date of notification of the decision to you by the Civil Aviation Authority and any evidence/information that support your application.

We collect personal information from you, including information about your:

We collect your personal information in order to facilitate your independent review application, and may use that information for our internal reporting and monitoring purposes. Your information will be stored in line with our Records and Information Management Policy and our obligations under the Privacy Act 2020 and Public Records Act 2005.

Besides our staff, in order to process your application, we may share this personal information as needed with:

  • the Civil Aviation Authority; and 
  • expert advisers at the request of the independent reviewer.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please contact us at independentreview@transport.govt.nz.

I understand that I may be required to provide information as requested by the reviewer regarding any matter at issue related to the decision under review, in accordance with sections 447(3)(a)-(b) of the Civil Aviation Act 2023.

I authorise the reviewer to make any necessary enquiries at their discretion about information on this form and/or accompanying documentation from the Civil Aviation Authority, experts and other persons so that they can: 
- review the decision for which you are applying, and 
- provide a report/ recommendations to the Director of Civil Aviation once the review has concluded.

I understand that I will be charged $1,000 (excl. GST) per IRF review application for individuals, and $1,500 (excl. GST) per IRF review application in any other case (such as for organisations) to reimburse direct and indirect costs involved in processing this application for review, in accordance with clauses 6(a) and (b) of the Civil Aviation (Independent Review Function) Regulations 2025.
